For medical device software development ALM/PLM, it's one of the best systems out there. However, it is a bit pricey for small firms. One of the other issues is that it is sold through resellers so your experience will vary depending on who your reseller partner is.

It is a very complete system that has every feature that you could want in a fully integrated package. One of the key features for us is the Varients package which allows for a checkpointing or branching capability of requirements so that you can have multiple versions as well as package (project) dependencies. So you can have multiple projects that depend on a single module and just update that module and all of the other projects that use that module will then be updated in terms of their requirements and traceability. Additionally it has a decent ecosystem for integration and where it is lacking, they provide an API so that you can make your own!

The only downside in my mind is that it is expensive in terms of initial and ongoing costs compared to some other products that offer similar features although they are not nearly as well integrated into a single package as Polarion is.
